Two rivers pass through Shtip, the Bregalnica which is the second largest in Republic of Macedonia and the Otinja which bisects the city center. The hill Isar, with its early medieval fortress on top, dominates the city and provides for the common reference as "The city under the Isar".
The borders of the municipality expand on a territory of 583,24 km². At the census of 2002 the municipality had 47.796 residents (41.670 Macedonians, 2.195 Roma, 2.074 Vlachs, 1.272 Turks).
